CE Conference Form 2022


Registration and payment must be submitted/postmarked by August 26, 2022:

Registration includes: Lunch on Saturday catered sandwich or salad lunch (meat and vegetarian selections)
and Saturday mid-morning and mid-afternoon snacks; Sunday mid-morning snacks

AVMA Accredited VT Program Students: FREE. *Onsite registration please bring a school printout of
your current schedule OR your current paid tuition stub OR a current valid student ID.

• WyVTA and WyVMA Members: $75 Saturday only; $50 Sunday only; or $105 Saturday & Sunday

• Non-Members: $100 Saturday only; $75 Sunday only; or $125 Saturday & Sunday

**ONSITE & Late registration ONLY = $135. If postmarked or registered online August 27th
through September 9th or registering ONSITE (regardless of membership.)

CERTIFICATES: A complete and submitted CE Evaluation Survey is mandatory to receive a
certificate of completion.

Onsite Attendees: Complete your conference evaluation and turn it in to the conference registration desk and
receive your CE Certificate before you leave the conference. Thank you for attending!

Virtual Attendees: You will be emailed an evaluation to complete on SurveyMonkey within 24 hours of the
conference closure. Please complete it within 3 days. Each registrant with a completed survey will receive
his/her certificate via MAIL to the address we have on file for you within one week following the conference
along with any additional documentation from the speakers received during the conference.

*ZOOM LINK will be emailed by Friday, September 9. Please download the zoom.us app on the device you
will use for the conference. Ensure your Computer System is updated AND empty your temporary files
(cache) so you are ready to join the meetings. Viewing and audio works best if all other applications are OFF
(email/games/etc.) on your device. Have a cell phone ready in case you need assistance to call or text us.


Guest Speakers Bios:

Lance C. Visser, DVM, MS, DACVIM (Cardiology)
Associate Professor of Cardiology
Department of Clinical Sciences
Colorado State University

Dr. Visser is from Grand Rapids, Michigan. He obtained a BS in 2007 and combined DVM/MS in 2010 from
Michigan State University. He then completed a small animal rotating internship at North Carolina State
University in 2011. He completed a combined residency/MS program in Cardiology at The Ohio State
University in 2014. Dr. Visser recently joined Colorado State University where he is an Associate Professor of
Cardiology. Prior to this, he was at the University of California, Davis (2014 - 2021). His primary research
interest is improving the noninvasive assessment of cardiac structure and function with an emphasis on the
echocardiographic assessment of the right heart.

Richard Ford DVM, DACVIM, DACVPM (Hon)

Following graduation from The Ohio State University, Dr. Richard Ford practiced small animal and equine medicine for 3 years. Subsequently, he completed an internal medicine residency at Michigan State University, then joined the faculty at Purdue University College of Veterinary Medicine before becoming a
professor of medicine at NC State University College of Veterinary Medicine, where he is an emeritus professor. He also recently retired from the Air Force Reserve as a brigadier general, where he was assigned to the Office of the Surgeon General at the Pentagon. Dr. Ford is a coauthor of both the American Animal
Hospital Association Canine Vaccine Guidelines and the American Academy of Feline Practitioners Feline Vaccine Guidelines. For almost 15 years, he has provided continuing education seminars to veterinarians on infectious diseases and immunization. He has published more than 200 papers and textbook chapters
pertaining to infectious disease in companion animals.

Josh Vaisman, CCFP, MAPPCP

Josh believes all veterinary professionals deserve to feel fulfilled and energized in their work, each and every day. Through his company, Flourish Veterinary Consulting, he combines more than 20 years of veterinary experience, a Master’s in Applied Positive Psychology and Coaching Psychology, education in positive
leadership and positive organizational scholarship, and a passion for guiding leaders to cultivate workplaces in which people thrive. When he’s not crafting ways to elevate the veterinary community, Mr. Vaisman spends his time with his wife, Greta, and their many creatures in Firestone, Colorado. To fill his bucket, he’s a soccer-playing, weightlifting, cookie eating beekeeper.

When you pick up your registration packet at Saturday morning check in, you will select one of the following
for lunch that day. Save time and have your choices reviewed before your arrival.
*Sandwiches are served with choice of French fries, onion rings, soup of the day, or salad.
*Salads come with your choice of dressing: Ranch, Blue Cheese Italian, French, Thousand Island, or Raspberry Vinaigrette

• Build your own Burger: ½# burger patty with your choice of cheese, bacon, sauteed mushrooms, caramelized onions, jalapeños, or an egg cooked to your liking.

• California Chicken Sandwich: Grilled or fried chicken breast topped with crisp bacon, avocado, and pepper jack cheeses.

• Saratoga Salad: Crisp lettuce, cucumbers, diced tomatoes, red onion, and green pita chips served with your choice of sirloin steak, grilled chicken, blackened salmon, or jumbo sauteed shrimp.

• Jamie’s Buffalo Chicken Wrap: Fried popcorn chicken tossed in our house made Buffalo Sauce, shredded cheddar cheese,
and lettuce wrapped in a jumbo tortilla wrap.
